INITIAL COMMENTS
 
This report is a result of an on-site complaint investigation conducted on February 24, 2026 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, QI5X6701 Blue Mountain Recovery was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the recovery house. Therefore, no plan of correction is required.
