Observations
Based on a review of six personnel records on January 14, 2009 and an interview with the facility director on January 15, 2009, the facility failed to document 1 hour a week of direct observation as a part of the close supervision of a counselor assistant during the first nine months of employment.

The findings include:

Six personnel records were reviewed on January 14, 2009 and January 15, 2009. One of six records documented a counselor assistant that requires close supervision for nine months. The personnel record and supervision notes failed to document one hour per week of direct observation since the counselor assistant's employment date of September 2, 2008.

Plan of Correction
Effective immediately, documentation that the Counselor Assistant has received close supervision of at least one hour weekly by a trained Counselor, or by the Facility Director, will be updated and placed in the Counselor Assistant's personnel file. This close supervision will continue for a nine-month period beginning in September, 2008. The Facility Director will monitor that supervision notes are completed and placed in the Counselor Assistant's personnel file.
