Observations
Based on client record reviews, the facility failed to provide a complete release of information form for an insurance company, CCBHO. The form included an "other" option under the specific information to be released section and the purpose section. In two of four records reviewed, there was a release form that indicated "other", but no further information was provided. Refer to client records #2 and 4.

Plan of Correction
On releases for CCBHO the life skills person checked the block for other and did not have the reason for other being checked.
A review was conducted by the Executive Director and the Clinical Supervisor on April 21, with the skills technician and the counselors to assure that releases will be written the correct way and that the option other is not checked unless there is a reason stated. This should not occur with a CCBHO release. |
