INITIAL COMMENTS
This report is a result of a supervisory review of the written report of the findings of an on-site licensure inspection that was conducted on March 13-15, 2019, by staff from the Division of Drug and Alcohol Program Licensure. Based on the supervisory review of findings from the on-site inspection, Pinnacle Treatment Center d/b/a Greenfield Counseling, was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this supervisory review:	Plan of Correction
715.6(d) LICENSURE Physician Staffing (d) A narcotic treatment program shall provide narcotic treatment physician services at least 1 hour per week onsite for every ten patients
Observations The facility failed to have the narcotic treatment physician provide services of at least 1 hour per week onsite for every 10 patiens for twelve weeks of sixteen weeks.Physician time sheets and weekly census reports for the months of November 2019 to February 2019 were reviewed on March 13-15, 2019. Census were reviewed for Wednesday to Wednesday. The physician hours were insuffiecient for the weeks of November 14, 28, December 12, 19, 2018. January , 9, 16, 23, 30, February 6, 13, 20 and 27, 2019.These findings were discussed with facility staff on April 25, 2019.	Plan of Correction M025 Executive Director will on an ongoing basis adjust facility Medical Director onsite hours as census changes to maintain the 10/1 ratio
