This report is a result of an off-site complaint investigation conducted on 8/31/23 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Za'kyah House was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

§ 717.29(g)(1) LICENSURE Physical plant standards - Heating-Cooling
§ 717.29. Physical plant standards.
(g) Heating and cooling. The licensee:
(1) Shall maintain an indoor temperature in the drug and alcohol recovery house between 65°F and 90°F at all times.

Observations
During a telephone interview with house manager, it was determined that the recovery house failed to maintain temperatures between 65 and 90 degrees at all times.

THREE WINDOW AIR CONDITIONERS WERE INSTALLED IN THIRD FLOOR ROOMs, they were installed September 3, 2023. |
