Observations
Based on a review of client records the facility did not meet the requirements of this regulation. The required components for a psychosocial evaluation were not fully addressed in client records #1, 2, 3 and 4. This represented four of four client records reviewed. The components not fully addressed included coping mechanisms and support systems.

Plan of Correction
Dwight Evans, Facility Director, will discuss with Doug Luisi, Drug and Alcohol counselor how to identify negative and positive coping mechanisms, and support options. The summaries will be reviewed thoroughly, ensuring policy compliance immediately. The project is currently in full compliance as of December 12, 2007. Further, the Facility Director will ensure this standard through ongoing training of proper documentation via supervision. |
