INITIAL COMMENTS
This report is a result of an on-site licensure inspection conducted on March 19, 2014 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Pathway Services, LLC was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection.	Plan of Correction
704.11(c)(2) LICENSURE CPR CERTIFICATION 704.11. Staff development program. (c) General training requirements. (2) CPR certification and first aid training shall be provided to a sufficient number of staff persons, so that at least one person trained in these skills is onsite during the project's hours of operation.
Observations Based on a review of staff CPR and First Aid cards, the facility failed to provide a sufficient number of staff persons trained in CPR and First Aid onsite during the project's hours of operation. The findings include: Facility hours of operation are: Monday through Friday from 11 AM to 7 PM and Saturday from 10 AM to 1 PM. On March 19, 2014, the facility presented copies of staff CPR and First Aid cards. The facility employs the Project Director and two part-time counselors. Employee # 1, the Project Director, is the only employee certified in CPR and First Aid. The facility failed to provide a sufficient number of staff persons trained in CPR and First Aid during the project's hours of operation. Employee # 1 is the Project Director and provides CPR and First Aid coverage during the following work schedule: Monday 8 AM to 9:30 PM Tuesday 8 AM to 9:30 PM Wednesday 8 AM to 12:00 PM Thursday 8 AM to 9:30 PM Friday 8 AM to 5:00 PM Saturdays Employees # 2 and 3, both part-time counselors, are neither certified in CPR nor trained in First Aid. The facility failed to provide a sufficient number of staff persons certified in CPR and trained in First Aid during the following timeframes when employees # 2 and/or 3 worked: Wednesdays 12:00 PM to 7:00 PM -and- Fridays 5:00 PM to 7:00 PM The owner confirmed the findings.	Plan of Correction Employees #2 and 3, both part-time counselors, are registered for CPR certification and First Aid training. Both employees #2 and 3 are attending the classes on April 28th and May 4th. On May 4,2014 employees #2 and 3 will be fully certified in CPR and trained in First Aid. These certifications will be good for two years and every two years the Project Director and employees #2 and 3 will be recertified in both CPR and First Aid. The owner of the business will ensure that the corrective action is implemented by May 4, 2014. The owner will ensure that the Project Director and both employees #2 and 3 will remain fully certified in both CPR and First Aid for the duration of their employment at this facility.
