Observations
Based on a review of personnel and training records and a review of the facility summary report, the facility failed to provide documentation of HIV/AIDS and TB/STD training in one of one personnel records.
The findings include:
Nine personnel and training records and the facility summary report were reviewed on October 5, 2011. Since the previous survey, two personnel records were required to have documentation of HIV/AIDS and TB/STD training. The facility did not provide documentation of the required HIV/AIDS and TB/STD training for employee # 8.
Employee # 8 is a counselor and was hired March 22, 2010. HIV/AIDS and TB/STD training was to be completed within one year of the date of hire or by March 22, 2011. The facility failed to provide documentation of TB/STD training as of the date of the inspection on November 2, 2011.
This was discussed with and confirmed by the facility director on November 2, 2011.

Plan of Correction
The Program Director and Human Resources will work with the staff member who does not have the HIV/AIDS training and get the staff member to the next available training no later than March 2012. The Program Director will also review trainings with all staff during staff meeting and individual supervision sessions to insure that all staff will be compliant with training requirements. |
