INITIAL COMMENTS

This report is a result of an on-site licensing inspection conducted on Novemeber 15, 2011, by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site licensing inspection, Family Service and Children's Aid Society was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection.

Plan of Correction

704.11(c)(2)  LICENSURE CPR CERTIFICATION

704.11. Staff development program. (c) General training requirements. (2) CPR certification and first aid training shall be provided to a sufficient number of staff persons, so that at least one person trained in these skills is onsite during the project's hours of operation.
Observations
Based on a review of facility staff CPR/first aid certification cards, the facility failed to ensure that at least one person trained in CPR skills onsite while providing a service.

The findings include:

Based on a review of staff records on November 15, 2011, conformation from the Project Director, and an inspection of staff CPR/first aid certification cards, the facility failed to have any staff trained while transporting the clients.

Employee # 5 was hired on 12-07-2004. This support staff provides transportation for the clients by driving a van for service to and from the facility. The staff member failed to have CPR/First Aid training.

Plan of Correction
The Facility Director will ensure that employee #5, the transportation driver, has been scheduled to take CPR/First Aid training on or before February 29, 2012. This training is completed in-house by a staff member who is certified to teach CPR/First Aid by The American Red Cross. The Facility Director will submit a copy of the training certificate upon completion. A training data put into place by the agency a few months ago now makes it easy to track employees who have taken CPR training & when the training expires. Copies of all training certificates are kept in the main agency & can be easily viewed when licensing visits.

705.2 (4)  LICENSURE Building exterior and grounds.

705.2. Building exterior and grounds. The residential facility shall: (4) Store all trash, garbage and rubbish in noncombustible, covered containers that prevent the penetration of insects and rodents, and remove it, at least once every week.
Observations
Based on a physical plant inspection, the facility failed to store trash in covered containers that would prevent the penetration of insects and rodents.

The findings include:

A November 15, 2011 inspection of the exterior grounds revealed the facility failed to store refuse in covered containers as to avoid the penetration of insects and rodents. The facility had failed to provide lids on the two containers and there were two garbage bags filled with garbage on top of the containers. There was also garbage lying on the ground around the containers.

The findings were confirmed by the facility director.

Plan of Correction
On November 15, 2011,trash containers with lids were purchased for this facility by the Project Director & placed behind the building. All garbage will be placed in the containers with lids and there will not be any garbage lying around on the ground. The Facility Director will be responsible to ensure this deficiency does not recur by physically checking on a weekly, or not more often, basis. Staff has been reminded they must place all garbage in the cans & make sure lids are on securely.
