INITIAL COMMENTS

This report is a result of an on-site complaint investigation conducted on September 14, 2015 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site complaint investigation, Gaudenzia Common Groun was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

Plan of Correction

705.2 (2)  LICENSURE Building exterior and grounds.

705.2. Building exterior and grounds. The residential facility shall: (2) Keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well-being of residents, employees and visitors. The exterior of the building and the building grounds or yard shall be free of hazards.
Observations
Based on a physical plant inspection, the facility failed to keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well-being of residents, employees and visitors.

The findings include:

A physical plant inspection was conducted on September 14, 2015. The facility failed to keep the grounds of the facility clean, safe, sanitary and in good repair at all times as the windows and blinds throughout the facility were in need of replacement. The windows were pulling away from the building, had visible mold, rotted wooden frames, and chipped paint. The blinds were broken and/or missing slots.

The ceiling tiles in the first floor hallway outside the medication room were missing causing the wires to become exposed. The ceiling tiles in bedrooms #9 and #10 had water stains.

These findings were reviewed with facility staff during the licensing process.

Plan of Correction
Estimates had been procured, at the time of the inspection, and the resultant contract was, and is in place, for replacement and reconfiguration of existing windows and blinds, for precisely the reasons the DDAP inspection disclosed. The installation of the new windows is projected to have been completed as of 12/31/15. Responsibility and accountability for this project belong to Rick Farling, Regional Maintenance Coordinator as well as, ultimately, to Warren Hodgkiss, Program Director.

Ceiling tiles in the first (and second) floor hall ways (there are no tiles IN rooms 9 and 10; they are OUTSIDE the rooms' doors in the hall way) were damaged and/or missing due to a malfunction of the air conditioning system in several zones throughout the facility which occurred within days of each other, as was the damaged ceiling in room #1. The ceiling tiles have been replaced, and the ceiling in room #1 restored to its original condition.

705.6 (7)  LICENSURE Bathrooms.

705.6. Bathrooms. The residential facility shall: (7) Maintain each bathroom in a functional, clean and sanitary manner at all times.
Observations
Based on the physical plant inspection, the facility failed to maintain each bathroom in a functional, clean, and sanitary manner at all times.

The findings include:

A physical plant inspection was conducted on September 14, 2015. The facility failed to maintain each bathroom in a functional, clean, and sanitary manner at all times as the exhaust fans in the bathrooms throughout the facility were observed to have a build up of lint that had not been cleaned.

The section of the ceiling in the bathroom of bedroom #1 that was damaged when the air condition unit leaked needs to be repaired.

These findings were reviewed with facility staff during the complaint investigation.

Plan of Correction
Each week, the facility receives a thorough cleaning (GI) facilitated by staff and clients and inspected, finally, by staff on duty. The vents of the bath room fans have been added to the list of items requiring special attention, and will be included in the weekly inspection pursuant to GI. The ceiling on room #1 has been repaired/restored to its previous condition. Party ultimately responsible for seeing that these issues are regularly addressed is Warren Hodgkiss, Director.
