This report is a result of an on-site incident follow-up investigation conducted on October 4, 2016 by staff from the Division of Accountability and Program Improvement. Based on the findings of the investigation, it appears the incident was handled appropriately by Pyramid Healthcare, Inc.-Pittsburgh. Therefore, the facility was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
