704.11. Staff development program. (c) General training requirements. Observations
Based on a review of personnel records and the Staffing Requirement Facility Summary Report, the facility failed to ensure all staff persons received a minimum of 6 hours of HIV/AIDS training and at least 4 hours of TB/STD training within the regulatory time frame in three of eight personnel records reviewed.

Employee #6 was hired as an assistant director on January 4, 2005. The HIV/AIDS training and the TB/STD training were due to be completed no later than January 4, 2006; however, neither training was completed as of the date of the inspection.

Employee #7 was hired as direct support on December 14, 2021. The HIV/AIDS training and the TB/STD training were due to be completed no later than December 13, 2023; however, neither training was completed as of the date of the inspection.

Employee #8 was hired as a resource coordinator on November 17, 2017. The HIV/AIDS training and the TB/STD training were due to be completed no later than November 17, 2019; however, neither training was completed as of the date of the inspection.

The findings were discussed with facility staff during the licensing process.

Plan of Correction
Employees # 6 and 7 were provided with the web address and the names of the required trainings along with all staff. Deadline provided by Director for completion and employees will turn in training certificates and evaluations to be placed in their employee files. Director informed both employees to complete the on-demand training immediately and they will continue looking for the TB/STD training in either a virtual format or a location within close traveling proximity.

Director will follow up to provide assistance and remove any barriers to obtaining the training in a timely manner.

Employee #8 is no longer an employee of RHD.
