Observations
The facility failed to obtain an informed and voluntary consent to release information form prior to the disclosure of information in one of eight client records reviewed.
Client #6 was admitted on January 23, 2018 and was discharged on September 20, 2018. There was documentation of communication with a family member on April 23, 2018 and August 27, 2018; however, there was no consent to release information form on file.
The findings were reviewed with facility staff during the licensing inspection.

Plan of Correction
The project director and/or the facility director will ensure that all consent forms are reviewed prior to all family sessions and family contact. All counselors will have consent forms on hand in order to obtain consent prior to any contact. |
