Room # 205, 208, 209 and 211 had loose shower flanges.

Room # 214 was observed to have wallpaper peeling off the wall in multiple locations inside the bedroom. The bathroom had a loose shower flange.

Room # 215 was observed to have wallpaper peeling off the wall in multiple places with a black substance viewed on the back of the wallpaper and on the surface of the wall near the bedroom window. The bathroom had a loose shower flange.

Plan of Correction
To ensure ongoing health of living spaces including client rooms and bathrooms, we're including paint/wall paper on the walk-through inspections moving forward. For all items listed herein, repairs or maintainance has been completed for each. Additionally, to assist in faster identification of these deficiencies as they arise, we've installed a ticket drop-box on the lobby, accessible by all clients, along with development of a new form found next to that box. The form lists Client Name; Room Number; Date; Location; Description of Problem. It's titled Maintenance and House Keeping Requests. Meetings were held with all units to introduce the form, and details regarding it are being added to the client handbook. Process wise; the ED checks the drop box Monday-Friday and inspects each area identified on all the forms, confirms the need for immediate attention, and submits internal maintenance tickets to initiate that maintenance or repair. Despite monthly walk-throughs, oftentimes damage or ware/tare can occur between walk-throughs that client's know of but don't have a formal process for bringing to staff attention other than verbally stating it to whichever staff member they choose. This process formalizes and organizes it and will help in identifying area's of repair/maintenance in-between walk-throughs. Having the ED involved ensures appropriate follow-through on behalf of the house keeping and maintenance staff.
