INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on June 8th, 2017 by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention and Treatment, Program Licensure Division. Based on the findings of the on-site inspection, Gaudenzia Inc., Vantage House was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

Plan of Correction

705.6 (7)  LICENSURE Bathrooms.

705.6. Bathrooms. The residential facility shall: (7) Maintain each bathroom in a functional, clean and sanitary manner at all times.
Observations
Based on a physical plant inspection conducted on June 8th, 2017 between 1:00 p.m. and 1:45 p.m., it was observed that the facility failed to maintain the second-floor Bathroom/Shower room in a clean and sanitary manner. There was black mold growing in the ceiling of the second shower stall at the time of the inspection.

The findings were reviewed with facility staff during the licensing inspection process.

Plan of Correction
As per licensing regulations 705.6. Bathrooms. The residential facility shall: (7) Maintain each bathroom in a functional, clean and sanitary manner at all times.

On 6/8/2017 contractors inspected the areas in the ceiling that appeared to be mold. Results of this inspection determined that the air condenser was frozen which seems to be the cause of the moisture, due to its location. The contractor is due to return to the facility by 6/13/21017 to inspect the condenser and to ensure its full operation.

Once the condenser is fully operating the Physical Plant Manager for the Central Region will remove the areas containing the mold and these areas will be cleaned. In addition, the areas that are removed will be replaced by 6/30/2017.

In order to ensure that this deficiency does not recur, the Program Director will monitor the area for mold beginning 7/3/2017.
