Observations
Based on a review of the Staffing Requirements Facility Summary Report (SRFSR) form completed by the facility on February 4, 2013, the facility failed to ensure that staff caseloads remained at or under 35:1.

The findings include:

On February 4, 2013 and February 5, 2013, the SRFSR form completed by the facility was reviewed. The form listed eight counselors as having a caseload, employees # 4, 5, 6, 7, 8, 9, 10, and 11.

Employee # 4 had 14 clients and a caseload of 20:1, employee # 5 had 15 clients and a caseload of 14:1, employee # 6 had 36 clients and a caseload of 36:1, employee # 7 had 13 clients and a caseload of 12:1, employee # 8 had 17 clients and a caseload of 24:1, employee # 9 had 19 clients and a caseload of 17:1, employee # 10 had 26 clients and a caseload of 37:1.

The standard work week was 35 hours per week.

Based on the total number of hours per week that the facility reported the employees devoted to their clients, the total number of hours of the standard work week (35), and the total number of clients assigned to the following employees on February 4, 2013, employees # 6 and 10 exceeded the allowable maximum 35:1 caseload.

The actual client caseload is determined by dividing the Full Time Equivalent (FTE) into the actual number of clients. The FTE is determined by dividing the number of hours devoted to the client's treatment by the standard workweek.

The number of hours per week devoted by Employee # 6 to client treatment, as reported by the facility on the SRFSR from, was 35 hours per week. The facility reported on the SRFSR form that employee # 6 had 36 active clients on February 4, 2013.

Employee # 6 (35/35 = 1 FTE 36 clients/1 FTE = 36: 1 caseload)

The number of hours per week devoted by Employee # 10 to client treatment, as reported by the facility on the SRFSR from, was 25 hours per week. The facility reported on the SRFSR form that employee # 10 had 26 active clients on February 4, 2013.

Employee # 10 (25/35 = 0.714285 FTE 26 clients/0.714285 FTE = 37: 1 caseload)

Plan of Correction
Clients will be moved off of the caseloads of the two individuals referenced (Employee #6 and #10) to bring the ratio down to 35:1. Going forward, Caseloads will be monitored daily by the facility supervisor to ensure that the client to staff ratio does not exceed 35:1.
