Observations
During the course of the annual inspection on June 9, 2016, the facility failed to ensure a counselor received a minimum of 6 hours of HIV/AIDS training.
A review of personnel record # 5, who was hired on January 6, 2015, did not include the documentation.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
Facility Director will insure that HIV/Aids Training will occur during first year of clinical hiring, by creating form at hire to be jointly signed by hire and facility director. Clinical supervisor will attest training is complete during time frame by his signature and dating form. The employee has completed HIV aids Training and is awaiting certificate. |
