This report is a result of an on-site complaint investigation conducted December 28, 2021 and February 1, 2022 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Adappt House was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

The residential facility:
(2) May not permit in the facility heaters that are not permanently mounted or installed.

Observations
Based on a complaint investigation conducted February 1, 2022, the facility failed to utilize properly mounted or installed heaters.
An unmounted portable space heater was observed on the floor of the Facility Director's office during a complaint investigation conducted February 1, 2022.
