INITIAL COMMENTS
 
This report is a result of an on-site complaint investigation conducted on December 10, 2012 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site complaint investigation, the allegations against Gaudenzia, Inc. Vantage House were unable to be substantiated. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
