Observations
Based on a physical plant inspection the facility failed to maintain each bathroom in a functional, clean, and sanitary manner at all times.
The findings include:
A physical plant inspection was conducted on December 11, 2015. A broken pipe in the ceiling in the first floor bathroom caused a leak which in turn made the bathroom nonfunctional.
The findings were discussed with facility staff during the licensing process.

Plan of Correction
Program Director contacted Northeast Treatment Centers maintenance department on December 11th, 2015 regarding the leak in the facility. Maintenance staff arrived on December 11th and fixed the broken pipe in the bathroom, cleaned any excess water in the bathroom from the leak and returned the bathroom to functional. Program Director debriefed staff and all bathroom's within the facility will be monitored on a daily basis to ensure they are properly working. |
