This report is a result of a complaint investigation conducted August 28 - September 6, 2024, by staff from the Bureau of Program Licensure. Based on the findings of the complaint investigation, Gaudenzia Inc. Harrisburg Concept 90 was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

705.2. Building exterior and grounds.
The residential facility shall:
(2) Keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well-being of residents, employees and visitors. The exterior of the building and the building grounds or yard shall be free of hazards.

Observations
The facility failed to maintain the building in good repair as evidenced by water leaking from the second floor ceiling.
On August 28, 2024, during a complaint investigation, water leaking from the ceiling on the second floor was observed.
Two (one black and one blue) plastic storage bins were observed on the floor partially full of water in the hallway outside the conference room. The containers appeared to be holding water from an active leak in the ceiling. Water was also observed on the floor.

Plan of Correction
Maintenance was notified and repair was completed on the second floor leak.
HVAC company was contacted about the second leak and that was repaired on 9/12/24. |
