Observations
During the July 20, 2016 inspection, the facility failed to document the facility director completed at least 12 clock hours of training annually.
The facility director had three (3) hours of annual training documented for the July 1, 2015 to June 30, 2016 training year.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
Effective immediately, Facility Director will receive will receive mandatory 12 hours of training. This will be monitored and managed by the Senior Program Manager of Alcohol and Other Drugs Services. Effective Immediately all training certificates will be copied within 72 hours of course completion. The training files will be reviewed monthly along with supervision to ensure that all trainings are accurately documented and filed in staff training file. |
