This report is a result of a change of ownership request on May 23, 2023. Based on the findings of the application review, Spark Wellness, LLC was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this inspection and no plan of correction is required.
As this was the application approval, not all regulations were reviewed. During future inspections, all regulations will be reviewed for compliance.
