INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on December 10, 2020 by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention and Treatment, Program Licensure Division. Based on the findings of the on-site inspection, The Retreat at White Birch was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

Plan of Correction

709.53(a)(2)  LICENSURE Medication records

709.53. Client records. (a) There shall be a complete client record on an individual which includes information relative to the client's involvement with the project. This shall include, but not be limited to, the following: (2) Medication records.
Observations
Based on a review of seven client records, the facility failed to document if the clients received their medications or why they did not receive their medication in the Medication Administration Record (MAR) or Missed Medication Log in all seven client records.

These findings were discussed with facility staff during the inspection process.

Plan of Correction
Our facility failed to document if the clients received their medication or why they did not in the Medication Administration Record (MAR)for the seven clients reviewed.

To correct this deficiency, all medication given or missed will be documented either in the MAR or on the missed medication sheet located in the MAR. Documentation shall include medication taken, date and time or missed medication, and brief summary of why it was missed as well as date and time and signature.

To prevent re-occurrence of this deficiency, the Executive Director will review the MAR daily to be sure the MAR is being documented as the meds are being distributed to the clients.

The Executive Director held a staff meeting on 12/18/2020 with all Staff to discuss the need to document all medication, whether taken or missed and how we expect it to be documented in the MAR.
