This report is a result of an on-site recovery house renewal inspection conducted on December 19, 2023, by staff from the Bureau of Program Licensure. Based on the findings of the on-site renewal inspection, 107002 Charlie ' s Way Sober Living was found not to be in compliance with the applicable chapters of 28 PA Code, Chapter 717, which pertains to the recovery house.

§ 717.29(b)(1) LICENSURE Physical plant standards - Bldg-Exterior-Grnd
§ 717.29. Physical plant standards.
(b) Building interior, exterior and grounds. The licensee shall:
(1) Keep the interior, exterior, and grounds or yard of the drug and alcohol recovery house clean, safe, sanitary and in good repair and free of hazards at all times.

Observations
Based on the physical plant inspection the recovery house failed to ensure the exterior window on the back porch is safe and in good repair. The back porch window is broken with missing glass causing it to be unsafe and a hazard to residents.
These findings were reviewed with the recovery house staff during the licensing process.

Plan of Correction
Citation has been corrected, the House Manager replaced the broken glass on the back porch window on 12/28/2023.
House Manager will inspect windows in the recovery house daily for any cracke, or broken glass for safety issues and repair immediately. |
