Observations
Based on a review of client records, the facility failed to obtain an informed and voluntary consent from the client for the disclosure of information which included specific information to be disclosed, in one of seven records reviewed.
Client #2 was admitted September 27, 2025 and discharged December 2, 2025. A release of information was signed to a government agency on September 27, 2025 which did not include specific information to be disclosed.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
The Facility Director shall train counselors on regulation to obtain and document an informed and voluntary consent from the client for the disclosure of information which included specific information to be disclosed, in one of seven records reviewed. Staff shall be trained by 1/9/26. The Facility Director shall review to files per counselor weekly to observe compliance. |
