INITIAL COMMENTS
This report is a result of an on-site provisional license follow-up inspection conducted on January 7, 2025 by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, ARKVIEW RECOVERY CENTER was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:	Plan of Correction
709.52(a)(3) LICENSURE Support service type 709.52. Treatment and rehabilitation services. (a) An individual treatment and rehabilitation plan shall be developed with a client. This plan shall include, but not be limited to, written documentation of: (3) Proposed type of support service.
Observations Based on a review of inpatient rehab client records, the facility failed to document proposed type of support service on individual treatment plans in two out of five records reviewed. Client #22 was admitted on October 22, 2024 and was discharged on November 17, 2024. Client #26 was admitted on October 21, 2024 and was discharged on November 21, 2024. This is a repeat citation from the August 6, 2024 licensing inspection. These findings were reviewed with facility staff during the licensing inspection.	Plan of Correction The Program Director will conduct regular audits to promote accurate documentation of support services on individual treatment plans. Arkview will provide staff training on documenting support services, with completion by 2/4/25.
