This report is a result of an on-site recovery house complaint investigation conducted on June 12, 2025, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, 79 Blue Ridge Dr. LLC W4EI6701 was found not to be in compliance with the applicable chapters of 28 PA Code, Chapter 717, which pertains to the recovery house.

The licensee failed to keep the drug and alcohol recovery house free of insect infestation.

Plan of Correction
House manager was contacted on 6/3/2025 about bed bugs in the recovery house. House manager was informed client bought clothing in from a storage unit without running them through the heat machine. House manager called exterminator on 6/3/2025 and exterminator came out to treat on 6/3/2025 House manager was told by Exterminator that no furniture, mattresses or clothing needed to be destroyed (treatment will kill all bugs). Exterminator came back out on 6/10/2025 to treat again. Exterminator will return on 6/13/2025 to treat again and then on 6/20/2025 to complete treatment. House manager will make sure that each client is running clothing bought into house through heat machine before bringing them into the recovery house. House manager will continue to monitor for 30 days before claiming that treatment has satisfied the problem. House manager will continue with regimented exterminator treatment that has been in place for the last 6 years. |
