Observations
Based on a conversation with the clinical supervisor, the facility failed to document the required period of close supervision for one counselor assistant.

The findings include:

On September 7, 2011, a plan of correction follow - up inspection was completed for the April 5-6, 2011 on-site licensure renewal inspection. The plan of correction was completed for the staff member who did not meet the counselor qualifications. The facility corrected the non-compliance by transferring the staff member to the appropriate position of counselor assistant. The Plan of correction also stated, "The clinical supervisor will provide ..... direct observation, and documentation of supervision. This should be accomplished within 9 months."

During a review of the employee training record on September 7, 2011, there was no documentation of supervision.

A conversation with the clinical supervisor on September 7, 2011 confirmed the documentation of supervision was not completed.

Plan of Correction
The Clinical Supervisor has begun providing close supervision to the Counselor Assistant. The Clinical Supervisor will document/date each supervisor session ove the next 9 months in order that the Counselor Assistant may move into a Counselor position. The Program Director will oversee the above process
