Compliance with this regulation could not be determined during the plan of correction follow up inspection. Compliance will be determined during the next scheduled inspection. The original deficiency is listed below:Based on a review of personnel records and an interview with the project/facility director and the administrative manager, the facility failed to document the completion of 25 clock hours of annual training required for counselors in one of one personnel record. The findings include:Three personnel records were reviewed on August 1-2, 2011. One personnel record pertained to counselors. One personnel record required the completion of 25 clock hours of annual training. The facility failed to document 25 clock hours of annual training in personnel record # 3.Employee # 3 was hired on May 5, 2010. The facility training year is from July through June. Employee # 3 had 0 hours of documented training hours for the annual training year from July 2010 through June 2011.The project/facility director and the administrative manager were interviewed on August 2, 2011 at approximately 3:02 PM and confirmed that employee # 3 did not have any documented clock hours of annual training from July 2010 through June 2011.

Plan of Correction
