Observations Based on a review of four personnel records on July 7, 2020, the facility failed to ensure that a counselor assistant met the requirements for counselor within 5 years of employment. A counselor assistant who has accumulated less than 7,500 hours of employment during the first 5 years of employment will have 2 additional years to meet the requirements for counselor.
Staff #3 was hired on September 5, 2007 and promoted to counselor assistant on January 8, 2013. Staff #3 was granted an exception in 2018 and was allowed two additional years to meet the requirements of a counselor. The exception expired on January 1, 2020 and Staff #3 still had not met the qualifications of a counselor but was still employed as a counselor assistant at the time of the review of personnel records.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction On July 2, 2020 the corrective action was completed regarding staff #3 by the Founding President and CEO. Staff #3 was again made aware of the qualifications and time frames for a counselor. He was removed from the counselor assistant position and was moved to the academic department. Staff #3 was provided with a new job description and is employed as the Academic Director.
The office of the Founding President and CEO will ensure that all future staff in the counselor assistant position will meet the necessary qualifications and time frames for a counselor. |
