Observations
Based on a review of the employee personnel record, the facility failed to document the required period of close supervision for one counselor assistant.

The findings include:

Three personnel records were reviewed for the position of counselor assistant during an administrative review conducted on January 3, 2013. The counselor assistant represented in record # 8, was hired as a counselor assistant on May 29, 2012. The personnel record representing employee #8 contained a Bachelor of Arts degree, however there was no verification that the degree conferred was in a qualifying field. Based on the regulations, employee #8 is required to have three months of documented direct observation and nine months of close supervision. As of the date of inspection, there was no documented supervision for employee #8 as November 20, 2012.

The lack of documentation was confirmed by the Facility Director on January 10, 2013.

Plan of Correction
NET Human Resources Team Leader contacted the Regristar's office at the school employee # 8 attendedin order to gain written confirmation to verify the degree for Employee #8 on January 4th, 2013. However, verification of degree has been unable to be obtained as of yet. As such, Employee #8 will be demoted to Counselor Assistant High School level. Supervision of Employee #8 will reflect this change in title. Upon receipt of verification of degree, Employee #8 will be promoted to Counselor. Moving forward, NET HR personnel will ensure verification of degree of all future hires prior to official hire.
