Based on a review of personnel training records and an interview with the project/facility director, the facility failed to document 25 clock hours of annual training for each counselor for the July 1, 2020 through June 30, 2021 training year.Employee # 2 was hired for the position of counselor on March 31, 2014. Only 6 clock hours of training were documented for employee # 2 for the July 1, 2020 through June 30, 2021 training year. The findings were reviewed with facility staff during the licensing process.

Plan of Correction
Three Rivers Youth understands the need for professional development as it relates to staff training hours and quality services. In order to maintain compliance with the staff training hours per training year, Three Rivers Youth will ensure each staff has at least 25 hours per training year.

The Project/facility Director will monitor more closely training plans and hours completed during weekly supervisions and monthly quality assurance checks. The staff will maintain at least 5 training hours per quarter to ensure the 25 hours are complete by the end of the training year. As of date, Employee # 2 is on track to meet the 25 training hours for the current training year.
