This report is a result of an on-site licensure renewal inspection conducted on August 22, 2018 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Resources for Human Development Inc. Morris House was found to be not in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

Observations
The facility failed to document a physical examination for three out of seven clients. Clients # 2, 3, and 4 were found without a physical examination on the day of the inspection.
These finding were reviewed with facility staff during the licensing process.

Plan of Correction
The Director and Clinical Team will work to expand Morris Home's network of Transgender Competent Health Care Providers to ensure that physical exams are able to be completed in a timely fashion. The Director will review the DDAP regulations related to physical examinations at an all staff meeting by 11/1/2018. |

709.53. Client records.

Observations
The facility failed to complete client record of services in four out of seven client records. Clients # 1, 2, 3, and 4 did not have records of services during the inspection.
These finding were reviewed with facility staff during the licensing process.

Plan of Correction
The Program Director will work with RHD Corporate to make recommendations for incorporating a Record of Service form into our Electronic Health Record. Until this change can be made, the Program Director and Morris Home Clinical Team will re-instate paper Records of Service to be uploaded in the Electronic Health Record, effective 10/15/2018. |
