Observations
Based on a review of personnel records, the facility failed to ensure clinical supervisors, who had not functioned for 2 years as a supervisor in the provision of clinical services, completed a Department-approved core curriculum in clinical supervision in one of one applicable personnel record reviewed.
Employee #3 was hired as a clinical supervisor on August 9, 2023 and did not have documented clinical supervision experience prior to being hired. There was no documentation a Department-approved clinical supervision core curriculum in the personnel record as of the date of the inspection.
This finding was reviewed with facility staff during the licensing process.

Plan of Correction
Clinical supervisor will complete the approved clinical supervision curriculum by January 8, 2025. Clinical supervisor will provide the training certificate/s to the Project Director along with the HR department and this will be added to the clinical supervisor's training record. Project director will continue to meet with the clinical supervisor once weekly in order to ensure full compliance. The facility has added the core clinical supervisor training to the list of requirements for hiring a clinical supervisor who has not functioned for 2 years in a supervisory role. The Project director and the HR department will enforce this requirement for future hires. |
