INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on 12-15-2014 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Roxbury was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

Plan of Correction

704.6(a)  LICENSURE Clinical Supervisor Qualifications

704.6. Qualifications for the position of clinical supervisor. (a) A drug and alcohol treatment project shall have a full-time clinical supervisor for every eight full-time counselors or counselor assistants, or both.
Observations
Based on a review of the "Staffing Requirements Facility Summary Reports" (SRFSR) for all of the facilities in the Project, the Project failed to ensure that there was one full-time clinical supervisor for every eight full-time equivalent (FTE) counselors in the Project.

The findings include:

The SRFSR for the four facilities that make up the project list a total of 17 counselors and 3 counselor assistants (some full time and some part time) and two clinical supervisors. The total of the hours per week provided to the project by counselors equates to 12.125 FTE counselor positions within the project.

The SRFSR for the four facilities within the project revealed that no employees were listed as a full-time clinical supervisor. The SRFSR listed one of the two clinical supervisors as facility director; therefore they were unable to provide full-time clinical supervision. In addition one of those clinical supervisors was listed as serving in the role of a counselor, therefore they were unable to provide full-time clinical supervision.

This finding was confirmed with the project director during the administrative review.

Plan of Correction
In order to ensure that a full-time Clinical Supervisor is in place for every eight-full time counselors/ counselor assistants, the Facility will have the Clinical Director, Clinical Supervisor and OP Director serve as Clinical Supervisor. This will ensure full-time clinical supervision of all projects. Each Clinical Supervisor will be responsible for ensuring appropriate staffing levels to ensure that case loads are within guidelines.

In order to ensure compliance with client/staff and client/counselor ratios, full-time counselor vacancies across the project have been filled. Also, PRN (or as needed) counselor positions have been added to ensure coverage of caseloads in the event of staffing turnover. This takes effect on 1/6/15.
