Observations
Based on a physical plant inspection the facility failed to prohibit heaters that are not permanently mounted or installed.
The findings include:
A physical plant inspection was conducted on May 19, 2015 at approximately 3:30 pm. There was a space heater located in the clinical supervisors office.
The findings were discussed with licensing staff during the licensing process.

Plan of Correction
The Program Supervisor removed the space heater from the facility on 5/20/15.
Chapter 705 Physical Plant Standards reviewed and will be monitored for ongoing compliance and any potential issues reported and address in formal Monthly Regional Maintenance meetings.
