Plan of Correction
Plan of Correction ? L 0325, 28 PA Code § 717.29(b)(1)

ODAAT acknowledges the finding concerning the hole and water damage in the ceiling caused by the actively leaking HVAC cooling unit.

Corrective action:

Upon identification of the condition, ODAAT notified the property owner and requested immediate evaluation and repair by a qualified HVAC contractor. The affected area was restricted from resident access, as applicable, to reduce the risk of injury and further exposure to leaking water. Temporary measures were implemented to contain the water and protect the surrounding area until permanent repairs could be completed.

The HVAC cooling unit will be repaired to eliminate the source of the leak. After the leak has been corrected and the area is confirmed to be dry, the damaged ceiling materials will be removed and replaced. Any affected surfaces will be cleaned and treated, as appropriate, to address moisture-related concerns. The ceiling will then be restored, sealed, and painted. ODAAT management will inspect the completed work to verify that the area is clean, dry, safe, sanitary, in good repair, and free of hazards before it is returned to regular use.

Measures to prevent recurrence:

ODAAT will implement the following ongoing measures:

Management will conduct weekly physical plant inspections, including ceilings, walls, HVAC units, plumbing fixtures, and areas vulnerable to leaks or water damage.

Residential staff will report leaks, moisture, ceiling damage, or other physical plant hazards immediately to the Program Site Manager or Recovery House Director to inform property owner.

The property owner or maintenance contractor will be contacted promptly when a repair required.

Open maintenance items will be reviewed by management at least weekly until the permanent repair is verified as complete.

HVAC equipment will be included in scheduled preventive maintenance and seasonal inspections.

Photographs, invoices, contractor reports, and completed inspection or maintenance records will be retained as evidence of correction and ongoing compliance.

Monitoring responsibility:

The Recovery House Director and Program Site Manager will be responsible for monitoring compliance, reviewing the maintenance log, following up on unresolved repairs, and verifying that corrective work has been satisfactorily completed.
