704.11. Staff development program. (c) General training requirements. Observations
Based on a review of employee training records on January 30, 2018, the facility failed to ensure that all staff were trained with 6 hours of HIV/AIDS and at least 4 hours of tuberculosis, sexually transmitted diseases and other health related topics training using a department approved curriculum.

Employee #2 a counselor was hired on 1/15/16 and as of the date of the inspection had not completed TB/STD Training.

Employee #3 a counselor was hired on 3/25/16 and as of the date of the inspection had not completed HIV/AIDS or TB/STD Training.

These finding were reviewed with facility staff during the licensing process.

Plan of Correction
Both Employee #2 and Employee #3 have submitted Training Plans for 2018 to the Clinical Director. The Training Plan for Employee #2 includes TB/STD Training. The Training Plan for Employee #3 includes the HIV/AIDS Training and TB/STD Training. Both Employee #2 and Employee #3 will be submitting certificates of completion for each Training to the Clinical Supervisor.

The Clinical Director will be responsible for maintaining individual binders for each employee at the facility and ensuring that certificates and reflections for each training are included.

The Clinical Director will be responsible for ensuring that the corrective action is implemented and will develop a spread sheet to track all new employee hire dates, due dates for mandatory trainings and contact staff 6 months and 3 months prior to the trainings to ensure that they are scheduled and will be able to complete them within the regulatory time frames.
