This report is a result of an on-site licensure renewal inspection and for the approval to use a narcotic agent, specifically methadone in the treatment of narcotic addiction. The inspection was conducted on April 2-3, 2015 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Pa Care, Llc D/b/a State College Medical was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this inspection and no plan of correction is required.
