INITIAL COMMENTS

This report is a result of an on-site licensure inspection conducted on September 8 - 9, 2010 by staff from the Division of Drug and Alcohol Program Licensure.

Based on the findings of the on-site inspection, Holcomb Associates, Inc. was found not to be in compliance with the applicable chapters of 28 Pa Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on October 13, 2010.

Plan of Correction

705.28. Fire safety. (b) Smoke detectors and fire alarms. The nonresidential facility shall: (3) Repair inoperable smoke detectors or fire alarms within 48 hours of the time the detector or alarm is found to be inoperative.
Observations
Based on the review of the fire drill log, administrative documentation, and an interview with the clinical manager, the facility failed to repair inoperable smoke detectors or fire alarms within 48 hours of the time the detector or alarm was found to be inoperative.

The findings include:

On September 8 - 9, 2010 the facilities fire drill log and administrative documentation was reviewed.

The log documented on 5/5/10 and 6/12/10 that a manual alarm via intercom was used.

The log documented on 7/7/10 and 7/13/10 that the alarm system was in the process of repair...a manual system was temporarily implemented until repair was complete.

The log documented on 8/17/10 that a manual alarm system was used. The log also documented under problems or training needs observed "system inspected and the mother board needs to be replaced. The part was ordered and repair scheduled."

A review of the facilities administrative documentation noted that on September 1, 2010 the inoperable smoke detectors or fire alarms were serviced and operable.

An interview was conducted with the facilities clinical manager on September 9, 2010, and she confirmed that repairs were not done within 48 hours of the time the detector or alarm was found to be inoperative.

Plan of Correction
The Clinical Supervisor will set off the fire alarm at each monthly fire drill. At that time if the alarm has malfunctioned in any way, repair services will be sought immediately. The management staff will ensure the alarm is fixed within 48 hours of the time it was found inoperable.

This will ensure Holcomb Behavioral Health Systems' compliance with Licensure Standard 705.28 (b) (3).
