INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on June 22-23, 2010 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Alpha House, Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on July 25, 2010.

704.11. Staff development program.
(f) Training requirements for counselors.
(2) Each counselor shall complete at least 25 clock hours of training annually in areas such as:
(i) Client recordkeeping.
(ii) Confidentiality.
(iii) Pharmacology.
(iv) Treatment planning.
(v) Counseling techniques.
(vi) Drug and alcohol assessment.
(vii) Codependency.
(viii) Adult Children of Alcoholics (ACOA) issues.
(ix) Disease of addiction.
(x) Aftercare planning.
(xi) Principles of Alcoholics Anonymous and Narcotics Anonymous.
(xii) Ethics.
(xiii) Substance abuse trends.
(xiv) Interaction of addiction and mental illness.
(xv) Cultural awareness.
(xvi) Sexual harassment.
(xvii) Developmental psychology.
(xviii) Relapse prevention.
(3) If a counselor has been designated as lead counselor supervising other counselors, the training shall include courses appropriate to the functions of this position and a Department approved core curriculum or comparable training in supervision.

Observations
Based on a review of personnel records, the facility failed to document the completion of 25 clock hours of annual training required for counselors in one of one personnel records reviewed.
The findings include:
Five personnel records were reviewed on June 22, 2010. Three personnel records were counselors. Two personnel records were required to document the completion of 25 clock hours of annual training. One personnel record was not employed for the 2008-2009 training year. The facility failed to document 25 clock hours of annual training in personnel record # 4.
Employee # 4 was hired on April 2, 2007. The facility training year is from July 1 through June 30. The training year for July 1, 2008 through June 30, 2009 was reviewed. Employee # 4 only completed 11 clock hours of annual training in 2008-2009.
The Director confirmed on June 22, 2010 that the counselor did not have the 25 clock hours of training for the 2008-2009 training year.

Plan of Correction
Employee #4 will be scheduled to meet the 25 clock hours of annual training from July 1, 2009 through June 30, 2010 for the personnel record. This will be evidenced by certificates of completion for the annual 25 clock hours and placed in employee #4's Training Manual located in the Executive Director's office. |

The residential facility shall:
(5) Ventilate toilet and wash rooms by exhaust fan or window.

Observations
Based on a facility tour on June 23, 2010, the facility failed to provide an exhaust fan or window in a bathroom.
The findings include:
A facility tour was completed on June 23, 2010 at approximately 10:00 AM. The third floor bathroom B on the 439 side of the building did not have an operable fan.

Plan of Correction
The third floor bathroom B on the 439 side of the building's fan has been corrected and is currently operable. The Residential Director was responsible for taking the corrective action. |
