Observations
The facility failed to document that the clinical supervisor completed at least 12 clock hours of training annually for the 2016-2017 training year in one of one personnel record reviewed July 10, 2018,

Employee # 2, a supervisor, had 7 hours of documented training.

These findings were reviewed with facility staff during the licensing inspection.

Plan of Correction
Employee #2 will demonstrate 12 clock hours of training annually. This will be completed between the calendar year of January 1, 2018 to December 31, 2018. Documentation will be located in employee #2's Training file and it will be the responsibility of the Executive Director to account for its completion.

To ensure this deficiency does not recur, all staff will be given their specific number of training hours required on a training form. The staff will be required to submit the completed training hours in October for monitoring by the Residential Director and/or Executive Director. Any unaccounted training hours will be given 60 days to come into training compliance. This form, along with the certificates of completion, and evaluation of the training will become a permanent part of the staff members "Training Manual"
