The facility's form that is used for the initial and annual training plans for all individual employees was revised and contains a section listing the mandated trainings. This form is reviewed and revised annually by the employee and supervisor. The administrative assistant tracks the completion of this form and its placement in each employee's personnel record; and notifies the facility director and/or clinical director when the employee's completed plan is overdue.
