Observations
Based on observation during a physical plant inspection, the facility failed to ensure that there were no heaters located within the facility that were not permanently mounted or installed.
The findings include:
A physical plant inspection was conducted on August 5, 2015 at approximately 10:00 AM.
A portable space heater that was not permanently mounted or installed was observed in a counselor office on the third floor.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
The space heater that was found in the counselor's office during the facility inspection was removed on August 5, 2015. The program's staff has been informed about the space heaters and the fact that they are not permitted in the program. To ensure that this regulation is uphold, beginning September 2015 the director will conduct monthly office inspections and any space heater found will be removed. |
