Observations Based on the review of client records, the facility failed to document dates of completion for medical histories in six of six client records reviewed.
The findings include:
Six client records were reviewed on September 9, 2010. Medical histories were required in six client records, #1, 2, 3, 4, 5, and 6. Per the facility's policy and procedure manual, medical histories are to be completed within 10 working days of the clients admission date. In all client records reviewed medical histories were documented but the medical histories did identify the date they were completed. Therefore, it could not be determined if the medical histories were completed within the time frame documented in the facility's policy and procedure.

Plan of Correction The Medical Histories which are being done within the first 10 working days will be properly documented by the form including a date as verification of the completion date. The form will be updated by 10/1/2010 by the program director. |
