INITIAL COMMENTS
This report is a result of an on-site licensure renewal inspection conducted on November 8, 2016 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Hope Springs Behavioral Health was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.	Plan of Correction
709.91(b)(4) LICENSURE Intake and admission 709.91. Intake and admission. (b) Intake procedures shall include documentation of: (4) Consent to treatment.
Observations Six client records were reviewed on November 8, 2016. Based on this review, the facility failed to document a Consent to Treatment in all six records. These findings were reviewed with facility staff during the licensing inspection.	Plan of Correction Consent to Treatment will be added to all patient records currently in treatment as well as new patient intake packet. This will be done by 11/17/2016 by Clinical Supervisor. These charts and intake packets will be checked by Facility Director bi-weekly to ensure compliance.
