Observations
Based on observation during a physical plant inspection, the facility failed to ensure that all trash, garbage and rubbish was stored in noncombustible, covered containers that prevent the penetration of insects and rodents. The findings include:A physical plant inspection was conducted on 11/25/2014 at approximately 1:00 PM.The trash dumpster on the side of the facility was overfilled with trash and not fully covered. These findings were reviewed with facility staff during the licensing process.

Plan of Correction
In response to violation 705.2(4), Building Exterior and Grounds, the program house manager will ensure that the dumpster lids remain closed at all times. In the event of excess trash, the house manager will contact the trash hauling company for an additional pick up outside of the current Monday, Wednesday, Friday schedule. A training will be conducted on December 11th 2014 for all of the staff. The house manager will meet with the residents in charge of trash disposal to review the procedure. |
