A physical plant inspection was conducted on August 3, 2012. To prevent continued water leakage from the door into the facility, an exit door that is a third exit from the building was taped shut on the outside, not permitting the door to be opened from the inside. The exit sign had been removed and there was a notice on the door that this door was not an exit.
The facility director was present during the physical plant inspection and confirmed the findings.
