Observations
Based on a conversation with the facility director on May 4, 2023, project director on May 10, 2023, and review of the facility director ' s resume/transcript, the facility failed to notify DDAP that the Project/Facility Director was let go in December 2022 and that the facility director put in their place did not have the qualified degree for the position as the facility director.
This was discussed with the Project Director during our phone conversation on May 10, 2023.

Plan of Correction
As of May 22,2023, a new Project Director was appointed, and she is currently overseeing the Honesdale office. She has a Master's Degree in Psychology, 12 years in D&A, 11 years in non-D&A clinical, and 13 years supervisory experience. The individual that was in the position of facility manger in the interim has been placed in the position of UR professional.
In the conversation of May 10th, DDAP was made aware of the on coming Project Director's credentials so they would be on record and provided the surveyor with her resume.
Thus, the Project Director is currently overseeing the Honesdale office, until a qualified Facility Director can be obtained. |
